Reasons include: Person-in-charge was not in control of foodborne illness risk factors, person-in-charge was using a bucket of water and detergent to wash hands, the hand sink was filled with items, refried beans and salsa were made off-site by someone else, person-in-charge was cleaning dishes with unapproved sanitizer, lettuce and cheese in the reach-in cooler were not date-marked, there was a live roach crawling around the storage area above the hand sink, a table with a cloth over it was being utilized as a food prep area/surface, there were no test strips for bleach, wastewater was being drained via piping from the mobile unit onto a grass field and there was a build-up of grime on the floors, walls and ceilings.

Reasons include: Person-in-charge did not demonstrate proper knowledge of foodborne illness risk factors, raw chicken was stored above raw and cooked beef, cooked chicken drumsticks and cooked chicken mix were sitting out at room temperature for three hours with a temperature above the proper level, multiple food products in the low-boy unit were note date-marked, several raw meats were being stored in plastic bags, sanitizer wiping cloth was being stored on the prep table and not in sanitizer solution, there was a build-up of raw meat juices at the bottom of the reach-in cooler and the floors, walls and ceilings were in poor repair.
